The is a wonderful resource! It has sample prayers for various healing prayer ministry situation. These prayers are powerful and can be used as is or suitably modified as God leads and directs. I have found the prayers for breaking curses and dealing with generational bondages to be especially helpful.

2. Francis MacNutt. 1995. Deliverance from evil spirits. A practical manual. Grand Rapids, MI: Chosen Books.
Francis MacNutt has provided us with a very useful reference work here. The subject matter can be overly sensationalized and result in a lot of unnecessary fear. God is the victorious one, not the devil. Since the victory of Jesus’ death and resurrection the evil one can only work through lies and deception. The topics MacNutt covers include: Necessary background: clearing away misconceptions; the existence and kinds of evil spirits; curses and the power of false judgement; getting ready [for deliverance ministry]; how to pray for deliverance; setting places free; and, some final words on the baptism of the Holy Spirit. There is also an appendix with sample prayers. Easy reading and a great resource.
